Educational Objectives:

  1. Identify patients at risk for venous thromboembolism (VTE).
  2. Describe national clinical guidelines for the assessment, prevention, and treatment of VTE.
  3. Review the Joint Commission National Patient Safety Goals for Anticoagulation Therapy and Proposed Performance Measurements for VTE.
  4. Discuss the role of pharmacists in the management of anticoagulation.

Target audience: pharmacists and pharmacy technicians
The Texas Society of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program (156-999-08-078-L01-P) provides 1.0 contact hour (0.10 CEUs) of continuing pharmaceutical education. To receive a Statement of Participation, a program evaluation form must be completed and turned in at the end of the program.
